How This Collaboration Works
Under this partnership model:
- When a patient followed by SSMT Clinic requires home support, their information is quickly shared with Bien Chez Soi Outaouais to ensure timely and appropriate service planning.
- When Bien Chez Soi Outaouais caregivers identify a need for medical or nursing follow-up, patients are referred directly to SSMT Clinic.
This two-way coordination reduces administrative burden for families and eliminates the need to navigate multiple disconnected service providers. It ensures that care remains coherent, efficient, and patient-centered.
